I hereby apply for membership in the Florida Association of Professional Lobbyists. I understand that the By-laws and Code of Ethics of the Association will be provided to me within seven days of execution of this application. This application final and binding unless I notify the Treasurer of the Association at the address below promptly after receiving the By-laws and Code that I do not wish to continue to seek membership and desire the return of my membership fee. Pursuant to the Bylaws of the Association and subject to the provisions of this paragraph:
A. I agree to comply with both the letter and the spirit of the Code of Professional Ethics of the Association;
B. I agree to complete the initial educational course developed by the Association in a timely manner as determined by the Board and subsequently satisfy the continuing educational requirements established by the Board;
C. I confirm that I am a registered lobbyist in this state and am not an employee of a unit of government;
D. I confirm that I have not been convicted of a felony. I agree to participate in the dispute resolution process prescribed in the By-laws and to waive any cause of action on the part of any other member arising from that member's good faith participation in any disciplinary matter that substantially complies with that process.
